Civil status system uses new Id and resident card application forms that work by  the 2d barcode technology. This new process promotes faster transaction processing and reduces data entry errors by eliminating the need to manually enter data from handwritten applications.

 To apply for:

1- Birth and death certificates: Notifying the Ministry of Health of these particulars in the prescribed application form. Accordingly, they are recorded in the civil register and the certificates are issued.

2- Recording marriage and divorce: Notifying the Ministry of Justice in an application form, upon which they are recorded in the civil register.

3- Resident card for the expatriate residing in the Sultanate and working for the private sector:  The Manpower form is an application form for a residence card.

4-The application forms for Omani identity cards and residence cards for non- working residents and for expatriates working for the public sector are available at:

  1- Transaction offices at every civil status centre.

  2- Application clearance offices.

The civil status website provides the service of printing the application forms for identity card and residence card with the aid of a pre-paid fee card. This card is available at application clearance offices at every civil status centre and application clearance offices. Thus, the applicant can fill them at his convenience while at home or at work. The applicant can prints the forms for one of the following applications:

1- New application or an application for renewal of identity card or residence card.

2- An application for amending or correcting the data in the identity card or residence card.

3- An application for replacement of lost or damaged card.

How to print the application forms from the civil status website?

Please follow these instructions to print the application forms for identity cards or residence cards from our website:

1- Get a pre-paid fee card for printing the D.G. of Civil Status smart forms from the application clearance section of the civil status centers or from the nearest accredited application clearance centre.

2- Log on to the civil status website www.civilstatus.gov.om

3- Click on the link for printing identity card and residence card applications or click here

4- Read the instructions on the webpage for the smart forms service.

5- Make sure that your PC has (Adobe Acrobat).  If the software is unavailable on your PC, first download it and then install it.

6- Enter your data and make sure the information is correct.

7- Move to the next pages to enter data by clicking on the button ‘next’

8- In case there is an error in your data, click on the button ‘back’ to correct the error (don’t use the icon ‘back’ from tools on top of the page).

9- In case you move from one page to another and the Net is slow, be patient and don't click on the icon ‘Refresh’ on top on the page, otherwise you will lose all your data.

10- After entering your data, insert the number of the pre-paid card without leaving any space between the letters and numbers (make sure you don't confuse between the number zero and the letter ‘o’).

11- Your filled form will appear as PDF.  Make sure your machine is connected to a good printer with original ink. Then, click on the printer icon (top on the left), or you can save the form on your PC.
